Our office and staff is committed to excellence. We provide the highest quality and compassionate dental care. We understand your need and realize you may be a little nervous about your visit. It is our mission to provide you with a comforting environment while we give you the dental care you deserve.
During your first visit we want to know all about you!
We will review your medical history and take x-rays, as needed. The doctor and staff will meet with you to address any concerns and develop a treatment plan just for you.
Be sure to bring this to your appointment.
- A completed health history form
- Your insurance card
- HIPAA Form
Please note that regular appointments every six months are recommended and may last 50-60 minutes. Please make us aware of any cancellation or rescheduling at least a day in advance.
